The Thompson Center is working to refine our new patient referral process in an effort to reduce the complexities for our new patients and referring providers. Our referral form has been updated and is available for download by clicking here. If you are referring a patient to the Thompson Center, please complete the referral form and either email to email@example.com or fax to (573) 884-1151.
