Other Providers and Agencies

The Thompson Center is working to refine our new patient referral process in an effort to reduce the complexities for our new patients and referring providers.  Our referral form has been updated and is available for download by clicking here. If you are referring a patient to the Thompson Center, please complete the referral form and either email to thompsoncenter@missouri.edu or fax to (573) 884-1151. For a description of services please click here.

Once we receive a referral, we will contact the patient’s family or caregiver and let them know a new patient packet has been mailed to them.  Please note the patient history form must be completed and returned to the Thompson Center before an appointment can be scheduled.

Please see our FAQs about appointments at the Thompson Center.

Thank you for your referrals and we look forward to working with you and your patients.

If you have questions about the referral process, please call us at 573-884-6052 or 1-888-720-0015 (toll-free).
